We are working on converting the map to a new plugin so that users like yourself can upload new locations. Unfortunately, the data for each map is slightly different and is taking longer than anticipated. Apologies for the inconvenience this may have caused. I will announce when I have it converted. However, in the mean time I will see about getting the old map back up and running. 🙂
Edit: I have reactivated the map on that page you linked. Please let me know if you have any other questions.
In just a few days, you won’t need a form to add your information! We are underway in implementing a new map system, so please bear with us while we work. 🙂 I will be posting a new update on the site to ensure everyone knows when it is up. Thanks for you interest in THC!
